Agenda:

Transition is a very important topic for HCP especially for HCP dealing with chronic diseases which can have onset in children or young people. The transition is a progress of the young people getting adolescent, therefore something which has to be dealed with from every young people. But in young people this is even more complicate due to the chronic disease, the need for treatment and medical appointments and to “being different”. If the young people are not supported by family and HCP during this transition period and they have a bad transfer (= change of HCP from paediatric to adult services) the risk for non-adherence is very high.  ERN Rare Liver are perfect places to discuss about and improve Transition since Adult HCP, paediatric HCP and patients are united in the ERN Rare Liver.

  • Why is transition important in general for chronic diseases
  • Why is ERN RL perfect platform to improve Transition, notification of the ERN Transition Working group and meeting as well as the youth panel
  • What is known so far about negative impact on transition and transfer
  • What we know from other diseases
  • How can transition be improved, examples of transition programmes
  • What are the differents between pediatric and adult PSC regarding diagnostic, presentation, treatment, comorbidities and outcome (case reports)
  • What are particular aspects of transition in patients with PSC
  • What can the paediatrician do, what can the adult HCP do?
  • Summary and Take Home Message (“What can everybody do tomorrow in her/his clinic?)
