Paediatric to Adult Care Transfer in Rare Liver Diseases - Published in JHEP

The outcomes from the ERN RARE-LIVER workshop on healthcare transition, which was held in Ghent and hosted 72 participants from 13 European countries, are published. During the event, the challenges and opportunities in managing transfer journeys from paediatric to adult care for young people with rare liver conditions were explored. The findings highlight the key importance of the transfer itself and reveal critical aspects around informative, relational, and management continuity of care. The practical recommendations derived from this multi-stakeholder event can serve as a benchmark for improving transfer journeys worldwide, not only for liver diseases but also for other rare conditions.
