EASL Clinical Practice Guidelines on vascular diseases of the liver

The 2025 EASL Guidelines provide comprehensive, updated recommendations for diagnosing and managing major vascular liver diseases—including Budd–Chiari syndrome, portal vein thrombosis, portosinusoidal vascular disorder, and hepatic sinusoidal obstruction syndrome. They emphasise early recognition of underlying prothrombotic conditions, appropriate use of Doppler ultrasound and contrast-enhanced CT/MRI for diagnosis, and a structured, stepwise treatment approach tailored to each disorder. Management focuses on timely anticoagulation when indicated, use of endovascular or radiological interventions (such as angioplasty or TIPS) for non-response or complications, and referral for liver transplantation in advanced or refractory cases. This guidelines highlight the need for multidisciplinary care and close monitoring for complications such as portal hypertension and hepatocellular carcinoma.
