About the Working Group
The Transition of Care Working Group is a transversal ERN RARE-LIVER Working Group focused on improving the transition from paediatric to adult healthcare for young people living with rare liver diseases.
Transition is a planned, purposeful and gradual process that supports adolescents and young adults as they move from paediatric to adult healthcare services. It includes medical, psychosocial and educational preparation, while promoting autonomy, self-management and continuity of care. Transfer is the specific moment when care is handed over from a paediatric service to an adult service. Transfer is only one part of transition, but it is often the point where continuity can break down. The Working Group therefore focuses both on the broader transition process and on making transfer safer, clearer and better prepared.
The Working Group aims to support practical, person-centred, developmentally appropriate and equitable transition care across ERN RARE-LIVER centres.
Get Involved
Clinicians, nurses, psychologists, social workers, researchers, patient representatives, young people and patient organisations interested in transition from paediatric to adult care are welcome to become involved in the Working Group.
The group works on practical tools, transition standards, patient information, Youth Panel collaboration, cross-ERN activities, workshops and research to improve continuity of care for young people with rare liver diseases.
