The Gastroenterology and Hepatology Unit in the Foundation IRCCS Ca’ Granda Ospedale Maggiore Policlinico in Milan is an internationally recognized center specialized in the diagnosis, management and treatment of hepatic diseases, with a dedicated expertise also in rare hepatic disorders. Main areas of expertise in rare liver disorders include biliary/autoimmune diseases (primary biliary cholangitis, primary sclerosing cholangitis, autoimmune hepatitis, IgG4 cholangitis, biliary atresia) and rare vascular diseases (non-cirrhotic portal hypertension, Budd-Chiari Disease, hepatic veno-occlusive disease and non-cirrhotic portal vein thrombosis): for these conditions, the Gastroenterology and Hepatology Unit serves as a referral Center at a regional and national level due to the capability of multidisciplinary care and integrated management of these complex disorders. Patients’ care is provided through a collaborative multidisciplinary team involving also the Radiology, Endoscopy and the Liver Transplant surgery Units, which allow a comprehensive management of rare liver disease and complications, included emergency care (i.e. early Transjugular Intrahepatic Portosystemic shunting). The Gastroenterology and Hepatology Unit is an authorized prescriber of specific medical treatments (i.e. obeticholic acid in primary biliary cholangitis) and is actively involved in international clinical trials. It collaborates also in national and international registries for rare liver diseases, such as Italian and Global PBC study Groups, the VALDIG international registry for vascular liver diseases. The Gastroenterology and Hepatology Unit is referral Center for the development of the Italian registry for non-cirrhotic portal hypertension. Concerning scientific activity, the Gastroenterology and Hepatology Unit is involved in single-center and collaborative studies of translational and clinical research about rare liver diseases, resulting in publications on distinguished peer-reviewed journals in the expert field. The equipe of the Gastroenterology and Hepatology Unit regularly participates in the development of Baveno Consensus Conference Guidelines for vascular liver diseases.
